read, understand and agree to the Internet Access and Service Agreement (\u201cTerms of Service\u201d) for MetaLINK Technologies, Inc. (\u201cMetaLINK\u201d). THIS TERMS OF SERVICE INCLUDES AN ARBITRATION REQUIREMENT THAT WILL HAVE LEGAL CONSEQUENCES THAT WILL AFFECT YOUR LEGAL RIGHTS AND WILL ELIMINATE YOUR ABILITY TO BRING FUTURE LEGAL ACTIONS. By submitting your signature below, you are agreeing to ALL terms and conditions in this\u00a0<a title=\"Terms of Service\" href=\"https:\/\/www.metalink.net\/images\/regulatory\/Internet-Access-and-Service-Agreement.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Terms of Service<\/a>.<\/p>\n<p>All contracts for the Wireless Internet Access Service (\u201cService\u201d) are a 24 month term (unless otherwise noted on the customer account) and are subject to an early termination fee of $199.95. My signature below authorizes MetaLINK to deduct my payment from the billing account information that MetaLINK has on file. I understand that if any of the information regarding the selected payment method changes, that it is my responsibility to notify the MetaLINK billing department immediately. There will be a $25.00 NSF Processing Fee for each instance of returned payments, electronic or by check. Should I become delinquent on payments, MetaLINK reserves the right to process overdue charges automatically to the payment information on file. I expressly consent to MetaLINK processing delinquent amounts to the payment method provided.<\/p>\n<p>The Wireless Equipment Protection Program is automatically in effect on the signing date of this Service Order. To insure that I receive optimal service, I agree to notify MetaLINK of any additional equipment purchased by me and added to my wireless setup as some equipment may alter or interrupt my current service. It is my understanding that even if the installer set up an internal wireless router (or other internal equipment) that I purchase separately, MetaLINK is NOT responsible for supporting my internal wireless network. (MetaLINK personnel can remotely evaluate wireless connections to ensure signal quality between service location and access point). MetaLINK personnel have advised me to use a UPS (Uninterrupted Power Supply) or a surge suppressor for my equipment. Either product is available for purchase by me from MetaLINK or I may purchase one on my own.<\/p>\n<p>I have reviewed the installation of the Service by MetaLINK authorized personnel, have seen it in operation on my computer(s)\/device(s) and affirm that the installation has been completed successfully and to my satisfaction. All of my questions regarding the wireless installation have been answered to my satisfaction.
